REQUEST A DUPLICATE W-2G

To receive a duplicate W-2G issued by Ameristar, you must first submit a signed W-2G Request Form. If you wish to obtain duplicate W-2Gs from multiple Ameristar properties you must submit a separate request to each casino. Alternately, W-2G Request Forms may be obtained at the mychoice booth.

Please read the form carefully and fill it out completely and legibly. By clicking on the form fields with your mouse, you will be able to fill-out the form in your browser, however, you will still have to print out the form and deliver it (in person, by fax or by mail) to Ameristar for processing.

If you choose to submit your request via fax or mail you will be required to obtain a notary public signature in order to verify your identity. If you choose to present your request in person, you will need to have proof of identity. Please allow up to 2 weeks for your request to be processed.
